Listed below are the documents that we will need to complete your child's registration.
Provide verification of address/proof of residency
Acceptable documents: Lease/mortgage agreement or a current utility bill (must show correct & current physical address). PO Boxes will not be accepted without legal documents for exceptions to this rule.
Student's original birth certificate
Only required for first time public school enrollment (establish the age of student) & new Wisconsin residents.
Current immunization records (Wisconsin or other location)
To look up student(s) immunization records from Wisconsin, use this link:
Wisconsin Immunization Registry. Once the record is displayed, select print and this will give you an option to download. This saved file can then be uploaded in the registration.

Free/Reduced Price Meals
You may apply online (or paper application) for free or reduced price school meals. If you have questions during any part of the process, please contact Kristin Tassoul for a confidential consultation. Free or reduced qualifiers may also qualify for applicable school fees to be waived, which may require an additional form to be completed and returned.
