2024-25 NEW Students 4K/5K Registration

This section is only for a child(ren) that has never been enrolled in school.

NOTE: If you currently have a student enrolled in Southern Door School District and would like to add a new student, please follow the instructions found at "Current Southern Door Families."   

Important notes:
Registration can be completed online or in-person (please call 920-825-7321 to schedule).  If you are interested in scheduling a tour or have any questions, please call 920-825-7321.

New Student Enrollment Procedure
For families that are new to our district and do not have a username and password for Infinite Campus or any existing student(s) enrolled in Southern Door, please complete the Online Registration. After the registration is processed by our staff a username and password will be emailed to you.
 
Requirements for New Student Enrollment
If you are enrolling a student in the district, there will be an option to upload the following documents in the registration. Please have them ready before you begin the registration process.
 
Provide verification of address/proof of residency
Acceptable documents: Lease/mortgage agreement or a current utility bill (must show correct & current physical address).  PO Boxes will not be accepted without legal documents for exceptions to this rule.

Student's original birth certificate
Only required for first time public school enrollment (establish the age of student) & new Wisconsin residents.  

Current immunization records (Wisconsin or other location)
To look up student(s) immunization records from Wisconsin, use this link: Wisconsin Immunization Registry. Once the record is displayed, select print and this will give you an option to download. This saved file can then be uploaded in the registration.
